Outreach Community Health Worker
Job Overview
We are seeking a dedicated Community Health Worker to join our team. The ideal candidate will play a crucial role in promoting community health and well-being by providing support, education, and advocacy to individuals and families. Schedule will vary each week. With required shifts on Saturday.
Duties
– Collaborate with healthcare professionals to develop and implement community health programs
– Conduct outreach activities to connect community members with healthcare services
– Provide health education on topics such as preventive care, nutrition, and chronic disease management
– Assist individuals in navigating the healthcare system, including understanding insurance
– Utilize motivational interviewing techniques to encourage healthy behaviors
– Collect and maintain data on community health needs and outcomes
– Advocate for underserved populations to ensure access to quality healthcare services
Requirements
– Background in social work, public health, or related field
– Strong communication skills with the ability to engage in public speaking engagements
– Knowledge of medical terminology and healthcare systems
– Ability to educate individuals on health-related topics effectively
– Familiarity with data collection methods for monitoring health outcomes
– CHW experience required; Core Competency Training preferred
