CHW Audit | Site Visit and Process Improvement Plan

Everyday Life’s process has been used to audit and support process improvement or design with over 100 programs nationwide.  This process works within the Community Response System – meaning the success or failure is not with the CHW alone – we have to recognize that integration much like Team work involves everyone.

I have found time and time again that the 5 critical criteria areas for successful integration include: Role & Recruitment, Communication, Screening & Referral, Placement and Scanning/Improving.
