Training Options
Self-Paced – Class will be a mix of self-paced and 2 coaching hours.
Virtual – 5 live instructor led virtual training days and 2 coaching hours.
Training is a total of 40 hours
2 private one on one coaching sessions.
Participants will have access to the schedule immediately after registration to begin to discuss topics most pressing to their work.
5 Self-Paced or virtual learning lab days
The curriculum is designed to build upon foundational skills of front line health workers to provide culturally responsive services. Each of the five competencies are presented in modules that promote the development of skills and enhance the knowledge base of those already employed.
Health Center 101/ Intro to community based care
Orientation to chronic disease 101 for CHWs (Diabetes, HTN, Asthma, Cancer, Heart Disease, HIV/AIDS)
Clinical integration and team based care
Clinical documentation and dynamic communication in healthcare settings
Orientation to virtual care and remote patient monitoring
Advanced assessment and risk screening for health related social needs
Care coordination/population management for CHWS core concepts and practice
Health promotion/health education for CHWs core concepts and practice
Eligibility To enroll in the Clinically Integrated Community Health Worker Certificate program, participants must:
- Be experienced Community Health Workers who have completed a CHW Core Competency course.
- This program is ideal for those seeking to deepen their skills and knowledge in working in a clinical setting.
Certification Requirements- To maintain certification, an annual 1-hour coaching session is required.
- This ongoing support ensures that CHWs remain updated and continuously improve their skills.
